Glamorous Line Formation, Transconjunctival Lower Eyelid Blepharoplasty

The patient wants to make her eyes look bigger and obtain a gentle impression.
Many, especially female, patients want to make their eyes look bigger, but when doing so, it’s important to consider both the upper and lower directions separately.
To widen the upper eyelid, you can create a double eyelid crease through double eyelid suture or incision surgery. Improving the eye opening can be achieved through ptosis surgery. In some cases, removing excess skin with a sub-brow incision might also be beneficial.
In cases like this patient, where you want to make your eyes look bigger by lowering the lower edge of the eyse, procedures such as Glamorous Line formation or lower eyelid blepharoplasty are appropriate. Simply put, it’s a surgery that lowers the lower eyelid. By lowering it in a balanced way, both the iris and the sclera are exposed more, resulting in a softer and more charming appearance. The position to lower the eyelid is determined based on the patient’s desired shape, starting from the center of the pupil to the outer edge.
When lowering the lower eyelid, it is essential to ensure that the lower eyelid line is aesthetically pleasing.
This surgery tends to revert, so we usually lower the eyelid a bit more than desired initially. After about three months, the eye shape stabilizes.
The surgery can be performed either from the skin side or from the conjunctiva, which is the inner side of the eyelid. Even if the surgery is done from the skin side, if it is done carefully, the scars are not too noticeable. However, if done from the conjunctiva, there will be no scars on the skin. The differences between the two approaches are as follows:
1. Skin Side:
• Advantages: Excess skin can be adjusted, less likelihood of developing entropion (inward turning of eyelashes).
• Disadvantages: Scars will be on the skin surface.
2. Conjunctiva Side:
• Advantages: No visible scars on the surface.
• Disadvantages: There is a possibility of developing entropion.
For younger patients, performing the surgery from the conjunctiva side can avoid visible scars. For those with sagging skin or those who may develop entropion when the lower eyelid is lowered, the skin-side approach may be better.
