“I’m self-conscious about being slim with small breasts, and I want to make them as big as possible! But I don’t want my breasts to look obviously augmented with silicone.”
This is a common request from young female in Japan.
Indeed, if large silicone implants are forced into a slim individual, the edges of the implant may become visible, especially in the upper chest area, and the breasts might appear unnaturally close together, making it challenging to achieve a balanced look.
Additionally, because slim individuals generally have less body fat, inserting a large implant can make it more noticeable by touch.
“For all that, I still want to go as big as possible!”
For those individuals, I recommend “Hybrid Breast Augmentation,” which combines fat grafting with implant-based augmentation.
1. Fat Grafting Breast Augmentation
Advantages: Soft, increases with your own weight gain, natural appearance
Disadvantages: Difficult to shape, not all fat survives the procedure
2. Implant-Based Breast Augmentation
Advantages: Allows for shaping, guaranteed size increase
Disadvantages: The edges may become visible if too large, challenging to achieve a natural softness
Hybrid Breast Augmentation leverages the advantages of both methods while compensating for each of their drawbacks.
In other words, the implant is used to maximize breast size, while fat grafting is used to enhance the smoothness of periphery of the implant, which tends to look unnatural.
This particular patient requested the inframammary approach instead of the underarm. For slim individuals, when large implants are inserted, the new inframammary fold also shifts downward. Since it’s difficult to predict exactly where this new fold will form, performing breast augmentation using the inframammary approach for slim patients requires experience. The advantage of this technique is that there are no scars in the underarm area, allowing you to confidently wear swimwear and clothing that reveals the underarms.
Regarding scar healing, collagen production progresses for about three months after surgery, causing the scar to temporarily harden. However, over the course of six months to a year, the scar matures and becomes a white, soft, and less noticeable scar.
Breast reconstruction is the surgery to create a new breast after a mastectomy for breast cancer. I have been involved in breast surgery for many years as a reconstructive surgeon. Reconstruction surgeries vary widely, including implant-based surgeries and complex procedures involving tissue transplantation from the thighs or abdomen, connecting small blood vessels under a microscope. I apply this extensive experience to achieve beautiful and natural breast augmentation results.
